Pay with Paypal

There is a minimal transaction fee for the convenience of using PayPal that is charged to our practice. To off-set these fees, there is a 3 % transaction fee for electronic payments.

To avoid online transaction fees, you may send a check to our business mailing address within 7-days of the date of service: Neighborhood Nutrition, 2414 E. Beryl Avenue, Phoenix, AZ 85028.

Out-Of-Pocket Nutrition Rates

• Initial Assessment  - $138.00
  
• Follow Up Appointment - $118.00
  

Co-Payments – Pay 3 installments

There is a $2.00 transaction fee added to each copayment.

• $30.00 ($10 x 3 visits) + transaction fee = $32.00
  
• $45.00 ($15.00 x 3 visits) = $47.00
  
• $60.00 ($20.00 x 3 visits) = $62.00
  
• $75.00 ($25.00 x 3 visits) = $77.00
  

Cancellation (No-Show) Fee

• $50 (+ $2.00 transaction fee) = $52.00
  

*All Payments are due at the time of the visit. Payments that are not received within 7-business days will incur a $25.00 late fee.
